These cards put a graveyard back into a library and shuffle it, to refill a deck that is running out or to take away the cards an opponent wanted to reanimate or cast again. Some act on one graveyard, like Reminisce, Feldon's Cane and Thran Foundry; others on every graveyard at once, like Mnemonic Nexus.
Many of the most-played are wheels that shuffle hands and graveyards together and then draw seven, such as Echo of Eons, Commit // Memory and Timetwister, or big spells that refill you, such as Finale of Revelation. Ulamog, the Infinite Gyre shuffles its owner's graveyard into the library when it is put there, so it rarely stays in a graveyard long enough to be reanimated.
Rules note: an instruction to shuffle a graveyard into a library applies to every card in that graveyard, and in Commander, a commander that would go to the library can go to the command zone instead.

You put every card from your graveyard into your library, then randomize the library (rules 400.12 and 701.24a). Your graveyard is empty afterward and the cards come back as future draws, so a deck close to running out gets more turns before drawing from an empty library loses the game (rule 121.4).
Yes, with a card that targets a player: Reminisce, Thran Foundry, Learn from the Past and Cranial Archive can name an opponent, which answers a reanimation or flashback plan before it starts. A card always goes to its owner's library (rule 400.3).
If your commander is in your graveyard when it would be shuffled into your library, you may put it into the command zone instead (rule 903.9b). Usually it isn't there at all: a commander put into a graveyard can go to the command zone right away (rule 903.9a).
For a while. Shuffling an opponent's graveyard away at instant speed, with Learn from the Past or Mnemonic Nexus, answers what is there now, but the graveyard fills again. Some work only on your own: Elixir shuffles back your nonland cards and gains life for them, and Psychic Spiral makes a player mill that many cards (rule 701.17a).
